Dependent Verification Form
Approximately one-third of all students who complete the FAFSA will be selected, by the federal government, for a process called verification. The verification process requires that students submit tax forms and any other documentation supporting their income as it was recorded on the FAFSA. The Verification Form must be completed and signed by both the student and one of his/her parents in order to complete the verification process.

Student Degree Plan Form
Students who are submitting an appeal to have their financial aid reinstated after suspension are required to submit a degree plan which lists the classes they will need to complete in order to graduate. Additionally, any student who has completed a program of study, and plans to return to school for an additional degree, will need to submit an updated degree plan. A degree plan may also be required if a student is approaching the maximum timeframe in order to complete their program of study. Request for Professional Judgment
If your income has drastically changed since the 2009 year, you may request a review of your financial aid. This Extenuating Circumstances Form is required, along with any supporting documentation, in order to have your file reviewed.
