Dependent Verification Form

Approximately one-third of all students who complete the FAFSA will be selected, by the federal government, for a process called verification. The verification process requires that students submit tax forms and any other documentation supporting their income as it was recorded on the FAFSA. The  Verification Form must be completed and signed by both the student and one of his/her parents in order to complete the verification process.

Independent Verification Form

SAP Appeals

Students who are submitting an appeal to have their financial aid reinstated after suspension are required to submit a SAP Appeal form with required documentation of an EXTENUATING circumstance that prevented academic success in the past.  Please refer to the Satisfactory Academic Progress Policy for more information.

Request for Professional Judgment

If your income has drastically changed since the 2009 year, you may request a review of your financial aid.  This Extenuating Circumstances Form is required, along with any supporting documentation, in order to have your file reviewed.
