UACCB Financial Aid Forms
The Financial Aid Office will request that all students receiving financial aid complete a UACCB Student Data Form. You may print, complete and turn in this form at any time. 2007-2008 Dependent Verification Form Approximately one-third of all students who complete the FAFSA will be selected, by the federal government, for a process called verification. The verification process requires that students submit tax forms and any other documentation supporting their income as it was recorded on the FAFSA. The Dependent Verification Form must be completed and signed by both the student and one of his/her parents in order to complete the verification process. 2007-2008 Independent Verification Form Approximately one-third of all students who complete the FAFSA will be selected, by the federal government, for a process called verification. The Independent Verification Form must be completed and signed by both the student and his/her spouse, if applicable, in order to complete the verification process. Students who are submitting an appeal to have their financial aid reinstated after suspension are required to submit a degree plan which lists the classes they will need to complete in order to graduate. Additionally, any student who has completed a program of study, and plans to return to school for an additional degree, will need to submit an updated degree plan. A degree plan may also be required if a student is approaching the maximum timeframe in order to complete their program of study. Request for Professional Judgement If your income has drastically changed since the 2006 year, you may request a review of your financial aid. This form is required, along with any supporting documentation, in order to have your file reviewed. |
